For emergency and after-hours treatment, go straight to any Lexington Medical Center Urgent Care or the Lexington Medical Center Emergency Department.

I'm coming in for an appointment.

Update your forms.

Spend less time in the waiting room when you fill out your forms before your appointment.

Patient Information Form

You should update this every year, or when your information changes.

Arrive 15 minutes before your appointment time.

Come early to update any necessary information. If you arrive more than 10 minutes after your appointment time, we may ask you to reschedule.

Let us know as soon as possible if you have to cancel.

Please do your best to cancel at least 24 hours before your appointment. If you cancel three times without advance notice, we may dismiss you from our practice.

Payment will be due at the time of your appointment.

Payment is due the day of service, including copayments, coinsurance and any other charges. If your care isn't covered by insurance, you are entitled to a 40% discount.

I'm a new patient.

For less time in the waiting room, fill out your new patient forms before your appointment.

Get the forms.

I need to refill a prescription.

Call and leave a message with your name, birth date, prescription and strength, pharmacy name and number and your doctor's name. Allow 24 to 48 hours for your request. We won't contact you about the request unless there is a problem.

We cannot refill some prescriptions.

You may be required to see your provider before we can refill your medication. We also do not accept refill requests from local or mail order pharmacies — you must make the request for these refills.

I'm waiting on test results.

You'll receive your test results in 7 to 10 days by mail or telephone.

I'm waiting on a referral.

Allow up to 7 days for your referral to go through. Once it does, our office or the specialist's office will call you with appointment information. If you make your own appointment, please let us know as soon as possible so we can obtain any prior authorizations that may be required.

I want to request my medical records.

You should complete a release form if you’re requesting records for anyone but yourself: your company, attorney’s office, another doctor’s office, etc. You may need to come into our office before we can process your request.

Release of Protected Health Information Authorization

Complete this form to request your records.

I need a school excuse.

We can only issue school excuses for you or your child if you come in for an appointment. All excuses are given at your doctor’s discretion.
