I'm a coming in for an appointment.

Complete eCheck-in thought LMC MyChart before you arrive.

As soon as seven days before an appointment, you can complete eCheck-in through LMC MyChart. This will allow you to update information and electronically sign documents prior to your appointment to make the check-in process faster. You can also view, print and fill out forms prior to your arrival by visiting our Appointments page. 

Arrive 15 minutes before your appointment time.

Come early to update any necessary information. If you arrive more than 10 minutes after your appointment time, we may ask you to reschedule.

Let us know as soon as possible if you have to cancel.

Please do your best to cancel at least 24 hours before your appointment. If you cancel three times without advance notice, we may dismiss you from our practice.

Payment will be due at the time of your appointment.

Payment is due the day of service, including copayments, coinsurance and any other charges. If your care isn't covered by insurance, you are entitled to a 50% discount.

I need to update my records.

We will need to verify your information and insurance at each office visit. Keeping your personal and health information up to date will help us to better serve you.

I need to refill a prescription.

Request all prescription refills during your routine office visit with your provider. Please bring all your medications with you to your appointment to review them with your doctor and leave with all your needed prescriptions. If you run low on your prescriptions before your next scheduled appointment, please call our office and leave a message on the refill line. Allow up to 48 hours to process your request.

We cannot refill some prescriptions.

An appointment may be required before your prescription can be refilled. Monitor your medications closely to avoid running out of them. We do not accept refill request from pharmacies.

I'm waiting on test results.

You can receive timely notifications and view lab results from tests you’ve had done through LMC MyChart. Test results should be available within 7 to 10 business days.

I'm waiting on a referral.

Allow 10 business days to receive notification for routine referral appointments. Either our office or the specialty office will contact you with appointment information. All referrals deemed urgent by your physician will be handled the same day.

I have a question for my care team.

You can exchange messages with your providers and read doctor’s notes from recent visits in LMC MyChart. Keep messages short and to the point. We will get back to you as soon as possible.

I need a medical professional to fill out a form.

Due to the volume of forms we process, please allow up to 10 business days for the completion of your form. There may also be a charge for completing your form, or your provider may require that you come in for an office visit for assistance in completing your form.

 

I need a school excuse.

We can only issue school excuses for you or your child if you come in for an appointment. All excuses are given at your doctor’s discretion.
